Flexible Family Support Program
Flexible Family Support is an ACT Health funded program and is delivered at Carers ACT, Community Options and Community Connections. Flexible Family Support focuses on improving the quality of life of all members of the family. Target group – Family Unit
Eligibility Criteria
FFS supports:
- Family carers of children with disabilities.
- Family carers supporting ageing parents with high needs.
- Family carers experiencing change after an unexpected or traumatic event such as an acquired disability or chronic illness.
- Family carers from different ways of life including Cultural and Diverse Background and Indigenous Background.
Purpose of the program
Flexible Family Support aims:
- To provide long term support planning for the future and working closely with the family to improve their quality of life
- To develop a support network with a combination of community and natural supports such family, friends and neighbours.
Flexible Family Support is a combination of Case Coordination and Brokerage
Case Coordination can include:
- Long Term Support
- Exploring different supports
- Guidance and Advocacy
- Working side by side with the family
- Review of caring relationships
- Bringing balance to the whole family
Brokerage can include:
- Special Equipment
- Services that bring positive changes to the family
